AED Request Form
To request an AED please fill out the form below and submit to Chiefs@mcadems.com. There is a downloadable PDF available as well for submission. Either option will sucessfully submit your request. Funding for the program is limited and each submission is evaluated. Due to demand there is a waiting list at times. Once a submission is recieved and evaluated the requesting party will be notified of the next steps. Thank you for choosing to support the community and help save a life!
Missouri law requires that a person or entity acquiring an AED must “ensure the automated external defibrillator is maintained and tested according to the operation and maintenance guidelines set forth by the manufacturer”.
Marion/Ralls County Ambulance District requires the End User of a Zoll Plus AED to maintain the Zoll maintenance agreement/Online oversight while in possession of the AED. Doing so provides the End User with the proper criteria needed to have an AED on the property (as noted above). The cost of keeping the maintenance agreement in effect will be offset by Marion County Ambulance District replacing the defibrillator pads/batteries for free as they expire. Failure to renew the maintenance agreement on each AED shall result in the removal of said AED from the premises.
There is nothing due for the first year after receiving your AED. After that, the maintenance agreement will need to be renewed in one (1) or five (5) year increments.oes here
